I have voluntarily joined Extend’s travel program in Israel, East Jerusalem, and the West Bank. I am aware that this tour may enter areas under military rule and/or areas where political violence regularly occurs. I understand that no entrant’s security in Israel or the West Bank can ever be fully guaranteed. I understand that access to health care is usually good, but under some circumstances access to health care, including emergency medical care, may be difficult or impossible to immediately obtain. I understand that, while Extend will escort me to the nearest medical facility at all times, I am responsible for all medical costs associated with my Extend tour. I release Extend unconditionally from any medical issues that may arise during my program. I understand that the tour can be physically and psychologically difficult and stressful. I have disclosed to Extend any physical or psychological conditions that may affect my participation. I release Extend from any legal liability for such harms. I understand that pictures of me and/or my Extend tour group may be used for advertising purposes.

I understand that if I act in ways that are disruptive to the tour, or that pose danger to other tour participants, my Extend tour leaders have the right to discontinue my participation. If my participation is discontinued for these reasons, I will be charged the cost of my transit to West Jerusalem and no refund for my tour fee will be returned to me.I am aware that traveling in Israel, Jerusalem, and the West Bank involves serious risks.Specifically, I am aware that there is an increased risk of violence due to recent unrest. I understand that Extend has taken steps to minimize the risks of travel, but, nonetheless, Icompletely accept and assume personal responsibility for any and all risks of damage and personal injury which may occur during or resulting from my participation in this Extend tour, including, but not limited to those risks described above. In doing so, I do not excuse myself from liability for such risks my behavior may create for other Extend participants, or other individuals.

I also hereby release, discharge and agree to hold harmless Extend, their staff, board directors, donors, partners and successors from any and all liability, claims, demands, rights or causes of action, brought on my own behalf or by or for any other person, or by my heirs, executors, or assigns, for personal illness, injuries or death, or any damage to or loss of personal property which may occur en route to, during, from or as a result of my participation in Extend’s voluntary tour program.I will remain with Extend for the full program, unless otherwise arranged with an Extend staff member prior to my Extend tour. In signing this document I expressly release, discharge, and hold harmless Extend and the persons described above to the maximum extent permitted by law in any state, territory, district or country where litigation might arise.I have read the foregoing and sign it voluntarily. I am of sound mind and act of my own free will and without coercion or duress in signing this Waiver of Liability and Assumption of Risk. I am at least eighteen (18) years of age. I have read and understood all of the above statements.
